If there are some fields that you do not have the appropriate information for; leaving them blank is acceptable. We can work with you to obtain any information that we need to complete the request.
The most important information that you can provide is the Municipality, the road name\or parcel number, and the location of where your driveway intersects with the road (not just the location of the structure on the property.)
